Who directs follow-up?

Carolinas for Care directs guide follow-up and the use of signup information. Jonathan Leake is the initial technical custodian. Contact gina@carolinasforcare.org with privacy questions, corrections, or deletion requests.

What you provide

The form asks for first name and email. A mobile number, attendee type, and interests are optional. It does not ask for diagnoses or treatment histories. The site records your permissions, the wording shown, the displayed participating-organization roster, and the time of your choice.

Separate permissions

Guide-related follow-up, future Carolinas for Care updates, and participating-organization contact are separate choices. Optional choices begin unchecked. No vendor receives the full attendee list. Organizations added later do not inherit your permission. A phone number is not permission for marketing texts.

Remembered access

This browser uses a secure access cookie and a minimal local flag so you can return to the guide. The local flag contains no contact information. Saved sessions and reading lists also stay on this device. Access does not automatically carry to another device. If you clear browser storage or need help returning, contact Gina.

Change your choices

You can withdraw all email and organization follow-up permissions below on the browser you used to sign up. To change individual choices, return to the signup form in that browser or contact Gina. After withdrawing, you can use this browser to review your choices and sign up again. Email delivery and the organizer’s ongoing follow-up process are still being prepared.

If this browser has no saved signup, contact Gina to change your preferences.
